I followed your procedure and here's how it worked for me:
1.  AVG Anti-Rootkit did not find anything.
2.  Malwarebytes 1.33 found and removed the Antivirus 2009 program and registry entries.
3.  Spybot, well like you said "old reliable".  Since I was doing this on a friend's laptop, I figured might as well run it.</description>

		<description>I have been removing this from computers for the past few days. Not sure where the outbreak came from. It took quite a while because the computers would not boot all the way up, or would only partially open. Keep trying, reboot until it lets you all the way in. Have these files on a jump drive. 

AVG Anti-Rootkit

You'll have to kill whatever app that's running, lately it is spyguard.exe, over and over until you're able to copy the AVG file to the desktop and install it. The machine will reboot, and then when it comes up, run the rootkit remover and it should find multiple files. It should be pretty obvious which ones you need to get rid of usually something with TDSS in it. Check them all and tell it to remove them. It will reboot, and come up way faster. 
Install the mbam.exe, click on the update tab and update it, and run the scan. Remove everything it finds. Some will have to be removed on startup, so it may reboot. 

Then install the Spybot and do the registry backup, update the software, immunize and then Search and destroy, and it will probably find a few leftover pieces the others didn't get, and again, it may have to reboot to get some of them on startup. 

Thus far, this has worked on all of the computers I have used it on. Prior to that they were getting re-imaged because nothing was working to get rid of this obnoxious mother.

		<description>alas, the SmitFraudFix was blocked by my Symantec ( at least partly) and would not load.

All the Manual removal instructions were not accurate.  AV2009 was not in the add/remove, nor could I find any of the processes in Task Manager, nor was it in the program files</description>

		<description>it was helpful. thankyou. i could delete the program after doing end task as u explained then tried to run- regedit but it didnot work. so i found the antivirus folder in my C drive and did Shift+delete it directly. thankfully it is deleted</description>
